Confirm which assignment you are checking
Start with the employer, course title and assignment date or version if shown. A familiar title does not establish that the current assignment is the one you completed previously.

If you believe earlier training should satisfy the requirement, provide the previous completion date and ask the coordinator to compare it with the current assignment. Do not assume either that you must repeat everything or that you can ignore the task.

Identify the last successful stage
Work from the observation you can actually describe.

These are diagnostic questions, not claims that a particular fault exists.
Use the course’s instructions for any required score, number of attempts or finishing step. A rule from a different course is not evidence of the requirement here.
Preserve the details before trying again
Record the error wording and the screen or activity where it occurred. If the employer provides a supported browser or device procedure, follow it and note the result.
Change one thing at a time so you can explain what happened. Repeatedly restarting an assessment or resubmitting an activity without knowing how attempts are handled can make the history harder to interpret.
Ask whether a retry is appropriate if the consequence is unclear. There is no verified universal reset sequence that fixes every Paycom Learning assignment.
If the barrier concerns accessibility, identify that need directly. A request for captions or another format is different from reporting that the same content failed to load.
Send a report that identifies the unresolved result
A useful hypothetical message is:
I finished the final lesson in the assigned safety course today. The assessment shows a result, but the assignment remains incomplete. Is another activity required, or does the recorded result need review?
Add the exact title, date and relevant result through the employer-approved channel. You do not need to send passwords or unrelated personnel records.
